Objective:To compare the effect of sildenafil (Viagra) and placebo in the treatment of erectile dysfunction in the elderly.
Methods:Forty-three men with mild to severe erectile dysfunction (ED) were randomly assigned to sildenafil or placebo. The subjects were instructed to complete a questionnaire to identify any erections and, if they had at least one of the following signs of ED: mild/moderate ED, persistent or recurrent ED, or absent, and the presence or absence of one or more of the following: headache, flushing, dyspepsia, nasal congestion, and nasal discharge. If a patient had not responded to the questionnaire and had no erections, the subjects were informed to stop taking the medication and asked to return for further testing.
Results:The subjects in both groups were able to achieve and maintain an erection of at least 4 hours. The proportion of subjects achieving and maintaining an erection was greater in sildenafil (p<0.001) than in placebo (p<0.001). The incidence of a recurrence of erectile dysfunction was also greater in sildenafil (p<0.001) than in placebo (p<0.001). The mean difference in the time to ejaculation was 2.2 minutes (95% CI: 1.1, 3.5) in sildenafil and 2.8 minutes (95% CI: 1.4, 5.1) in placebo. There were no significant differences between the groups in the number of men who reported to have sexual intercourse or how often they reported sexual intercourse. The frequency of sexual intercourse in the two groups was similar.
Conclusion:Our data suggest that sildenafil, similar to placebo, may be useful for the treatment of erectile dysfunction in the elderly. Further studies are required to determine the optimal duration of therapy and to compare the effects of sildenafil and placebo.

In a sub-analysis of 3,079 patients who were included in this study, patients in the treatment group showed significantly higher CEDAR levels than patients in the placebo group (0.23 vs 0.07 mm Hg; P < 0.001), and the reduction in the incidence of CIDAR was significantly greater with sildenafil (0.16 mm Hg vs 0.07 mm Hg; P < 0.001) and sildenafil/viagra (0.20 mm Hg vs 0.07 mm Hg; P < 0.001) compared with the combination of sildenafil and placebo. These results suggest that sildenafil and the combination of Viagra and Sildenafil are equally effective in reducing the incidence of CHD-related CVD-AEs.
Sildenafil and sildenafil/viagra, both of which are selective inhibitors of PDE5, were found to be more effective in reducing the incidence of CVD-AEs than the combination of sildenafil and Viagra. These results were consistent with the results of the study by Cosson et al. [].
